Needed on Admission Day
Photo Identification your Health Insurance and Social Security Cards. If you do not have these forms of identification, bring in some mail with your current address on it. If there are problems with identification, please talk to us. We have a duty to establish that you are who you say you are, but we also have a duty to get you into treatment. We want to eliminate any obstacles in your path to recovery.
Intake Process
On the day of your admission please be prepared to spend 1 ½ hours or more with the Intake Counselor. That may seem like a long time, but addiction treatment is a very serious matter and we cannot treat you if we do not know about you. Also, please be prepared to spend up to an hour with the Nurse Practitioner or Physician. Again, we will be using medication to treat you, and we need to know about your medical history and current medical condition.
We will also be asking you to give us written permission:1. To obtain information from your last addiction treatment program (if any).2. To communicate with your Primary Medical Care Provider (if any).3. To communicate with your Psychiatrist or other mental health provider (if any). As long as you are clear-headed, we will start treatment with methadone on the same day. If you are intoxicated (from alcohol or any other drug) you will not be able to give informed consent for treatment, therefore, we will not be able to treat you. In addition, if you are intoxicated, it may not be safe to give you methadone.
